It finally overheated the steel in the boiler and it let go. The engineer and fireman died instantly. The head brakeman lived long enough to tell what happened. One has to really try or be stupid to explode one. It just doesn't happen without human help.

It happened quite often, this one doesn't look that bad really, sometimes they would crater the track under the engine.
