The news media identifies the names of the pilots of the crashed Asiana airliner. Yeah.

I can't believe adults could not detect this adolescent prank. The summer intern at fault will never work again in this industry, but at the same time will be an immortal God to all forever for pulling this insanity off on the "adults."

A screengrab.  I mean, really, what about those multiple layers of fact checkers and editors?

The key part of the apology:

The National Transportation Safety Board apologizes for inaccurate and offensive names that were mistakenly confirmed as those of the pilots. A summer intern acted outside the scope of his authority when he erroneously confirmed the names of the flight crew on the aircraft.

Hahahahahahahahahahahahahahahahahahaha!  This made my day.

Update: the editor that let this through has been identified: it's Seymour Butts.
