Last weekend's flood started to wash out the road just a ways from the house. It'll be interesting to see what the city does with this. Right now the sidewalk above is cracking, and those cracks have begun spreading to the asphalt. You can see the dip.
Looks like a good candidate for a replacement culvert made of fiberglass reinforced plastic.

That slump has nothing to do with the culvert. Slope plus gravity plus saturated ground did the work. The best thing would be a bridge with concrete revetments extended well to each side. Basically, eliminate the soil.
I should have taken a closer look. That abutment was already damaged. Maybe a heavy truck during saturated ground was the final straw.
