Something that would limit the truck to 5MPH until the dump was down and locked would do the trick. A back-up seat taser to get the driver's attention would also not go amiss.
Staged up on a loader spread, CAT 777 rock trucks, watched one pillbilly get loaded, drive to the rock dump, drive back 3.5 miles with the bed up, get back in the line up, about 25 of us not saying nothing, after about 20 minutes, bed still up, backs under the loader bucket, loader guy has to back out of the way, sits there about 5 minutes before the driver gets on the radio asking whats the hold up, nobody says a peep, driver few minutes later asks again why he ain't getting loaded, same thing not a peep out of anyone, takes another 5 minutes fir the jamoke to finally see his bed's raised. This is at a coal mine, driver is stoned out of his mind on pills. Just about killed me a week later cause he was texting in his phone, stoned again. Was "laid" off for saying to the foreman you got to do something, cause I'm not gonna be the guy this drivers wife learns I killed his husband in an accident. Turns out the general manager was getting his supply of pills from that driver. One night he tripped coming up the bus stairs, Was driving, not kidding, his igloo diner bucket fell and its contents rolled down the school bus flor, was crammed full with script bottles of Oxy's. That was some pretty effed up sheet.

Its called lack of driver situational awareness. Those trailer dump beds don't suddenly raise in a couple seconds.
DeleteFailure to keep dump trailers in the down and locked position means this is not all that unusual. The usual victims are power lines and bridges.
